Department Divisions - Public Health Nursing, Environmental Health, Vital Statistics

Vital Statistics:
All births and deaths occurring in the health district are filed with our department. All original certificates are on file at the Ohio Department of Health. Certified copies of certificates can be obtained at our department for a fee of $18.00. The department also issues burial permits. This division is very important to our department. We obtain much information on the health of our community from this data and this information aids us in planning for services to meet the needs of our community. We also provide a variety of information to other agencies and organizations from our computer database.  Please click on the following link for more information on how to obtain a certificate.   Request a certificate

Genealogy searches are another service of the vital statistics division. All births and deaths are currently cataloged by year in hard copy and listed by name in alphabetical order along with other categories of importance. Once the individual record is located it can be viewed on microfilm. This process makes record searching easier for the public but we hope to have all of our records computerized in the near future.
