ALLIANCE CITY HEALTH DEPARTMENT
Request a Birth or Death Record
All births and deaths occurring in the health district are
filed with our department. All original certificates are on file at the Ohio
Department of Health. The department also issues burial permits.
You may obtain a certificate by one of the following three ways:

Certified copies of certificates can be obtained at the
Alliance Health Department, 537 East Market Street, Alliance, Ohio 44601
between the hours of 8:30am - 4:30pm Monday through Friday. The fee
is $18.00 per copy.

You may purchase a certified copy by sending a request
to the Health department with the following information: name, birthdate,
and parents' names of the person you are requesting the birth
certificates for or name, date and place of death if requesting a death
certificate.
A Money order payable to Alliance Health Department must
accompany the request. No personal checks will be
accepted. Please send a return address along with
the request for certificate.

You may purchase by Mastercard or Visa credit card (no debit cards)
either by e-mail at
hkouskouris@alliancecityhealth.org or by telephoning (330) 821-7373,
ext.11 with the following information: Visa or Mastercard #,
expiration date, cardholder's name and address (no P.O Box), phone number,
name on record, date of birth or death, and where the certificate is to be
sent. The fee is $20 per copy if using a credit card.
